11/4/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client appeared to be in a content mood evident by his facial expressions such as a smile on the face. QMHP-C engaged client as well as mother in a discussion about his behaviors since the last session. Mother verbalized to QMHP-C on how she has been having issues with client being disrespectful to her when she ask him to complete certain task 2-4 times a week which occurs on a daily basis for at 20 minutes. Intervention: QMHP-C redirected Client and reminded him that the behavior that he was showing was disrespectful. QMHP-C demonstrated Client's reactions and discussed with him the proper way to behave when being spoken to by someone who is an authority figure. QMHP-C reminded Client the importance of respecting adult figures, especially his father. QMHP-C role played with Client and gave him insight on how to better respond to a situation when he is frustrated. QMHP-C encouraged Client to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP-C listened to father after several attempts to get his attention at which QMHP- C will assist client with these skills at least 3-4 times weekly. Response: Client was able to select topics that were directly related to reduction of stress and emotional negativity, which made him less agitated and more relaxed. Client expressed that he hates being in the house all day but understands why he must remain inside. Client displayed little concern about his health and assured that he was taking the precautions to stay clean and proactive. Client began separating himself from toxic environment in order to decrease tension in the household. Client is observed correcting his mistakes and making good choices. Client was able to utilize coping skills to eliminate stressors and unwanted feelings. Client displayed self determination to stay focused and pay attention in session. Client utilized meditation and relaxation techniques. Client was seen decreasing anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Client was observed avoiding disruptive and impulsive actions demonstrated when Client proceeded to walk away and utilize deep breathing.
